· Both parents must be in agreement to raise their child in the Catholic faith.

· The Birth Certificate of the child to be baptized is required.

Both parents or legal guardians and both Godparents must participate in a Pre-Baptismal class:
If you attended class at Saint Thomas the Apostle: Kindly provide the month and year when you attended the class. If you can’t remember or your name doesn’t appear on the attendance list,
you will be asked to attend the pre-baptismal class again.
